Summary of Position Serves as case manager for medically complex, intermediate, skilled care including Medicare clients care units with a specialized caseload of clients with multiple psychiatric, substance abuse, behavioral and medical problems. Serves as liaison/advocate between veterans, family/responsible party, facility disciplines, other agencies and institutions. Interviews and establishes rapport with veteran and their family/responsible party to assess individual and family/responsible party roles. Conducts ongoing counseling sessions with veterans and their family/responsible party to assess individual and family/responsible party inter-relationships to identify specific diagnosis related to social services and counseling needs. Provides information on condition or progress of veteran to family/responsible party as warranted. Remains in close contact with veteran's family/responsible party by phone as needed, returning voice mail inquiries throughout the work day. Provides individual supportive counseling to veterans with medically complex conditions, including, but not limited to terminal illness, mental illness, dementia, Alzheimer's disease, substance abuse, post traumatic stress disorder. Assesses and documents therapeutic progress and actions taken to resolve problems or conflicts. Remains apprized of veterans progress, actions and behavior to validate the assessment criteria. Documents problems, incidents and actions taken or any pertinent information in medical charts as needed. Provides supportive therapy to family/responsible party as needed. Accesses, updates, and inputs into MDS computer program, complex, individualized care plans to address the veteran's psycho social needs, problems and behavioral/emotional conditions, cognitive level, and signs care plans for skilled care and intermediate nursing care residents including Medicare residents, through relevant information gathered from personal observations and interviews with the veteran. Prepares correspondence with community agencies and serves as primary liaison person with all outside agencies such as courts, schools, foster care and legal authorities. Assesses the level of care and intervention, including discharge, reassignment, change of treatment strategies and techniques; communicates these findings to the interdisciplinary treatment team. Attends and participates in community health-related programs, in-services, continuing education, and maintains professional reading. Develops a working knowledge of correctional or community resources in order to recommend or refer offenders for special services. Provides psychosocial case management to individuals, groups, and families when this intervention is determined to be most appropriate.
