Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations._ **Responsibilities:** GENERAL PURPOSE The social worker for SBHC provides psychosocial assessment, psychiatric diagnosis and treatment for patients 10-21 years old in an urban school-based clinic. The social worker provides continuous and comprehensive services necessary for the promotion of optimal mental health, prevention and treatment of behavior disorders and health maintenance. This includes the assessment, diagnosis and management of mental, emotional, behavioral, addictive, and developmental disorders and disabilities. The social worker works collaboratively with other onsite medical and mental health providers as well as with community providers and school personnel. ESSENTIAL FUNCTIONS + Provides screening, evaluation, individual, family and group treatment and uses evidence-based interventions with the patients and their families to influence patient outcomes. + Performs psychosocial assessment and diagnostic formulations to adolescents, including administration and interpretation of tests and measures of psychosocial functioning. + Works collaboratively with a physician and/or mental health provider of record to provide comprehensive adolescent behavioral health services to students enrolled in the school-based clinic. + Ensures the development and implementation of comprehensive and individualized treatment plans for patients and refers patients for further care and services within the school clinic, the school building and or larger community. + Provides social work treatment to patients and families within SBHCand identifies learning needs of various populations and contributes to the development of educational programs to meet those needs. + Maintains a customer-oriented approach to meeting patient needs in an expedient manner, involving the family in the plan of care as appropriate. + Documents treatment in patient’s electronic medical record in compliance with Division, Department, and Hospital Policy. + Attends Social Work Division, Social Work in Psychiatry and Program staff meetings. + Participates in activities of the Social Work and Psychiatry Division Psychiatry Division including, but not limited to, ensuring policies and procedures are implemented with regard to professional practice, discharge planning and patient care, timely completion of time reporting and other Hospital or Division forms; Hospital and Division mandatory in-service training programs and UHS reviews. + Participates in routine committee process of the Social Work Division and facilitates Division and Hospital quality assurance and staff development activities. + Subscribes to the departmental goal of creating a culturally competent environment by treating patient, families, trainees, and co-workers in a sensitive manner with appropriate attention to cultural differences. + Participates in available cultural competency events and training appropriate to job duties. + Identifies system specific problems, which require evaluation and/or change; works with appropriate staff to resolve these issues by participating in performance improvement processes. + May provide back-up as requested and able to other units/programs during the absence of certain co-workers. Other duties as assigned MINIMUM EDUCATION & EXPERIENCE + Master’s degree from an accredited school of social work (CSWE) REQUIRED + 2-3 years professional social work practice beyond the master's degree, at least one of which must include psychiatric emergency, crisis or outreach work. + or an equivalent combination of education and experience and working with adult populations. + Prior experience in providing individual or group therapy in an acute clinical setting. PREFERRED KNOWLEDGE, SKILLS AND ABILITIES + Knowledge about and ability to provide behavioral health assessments/evaluations and evidenced based interventions. + Commitment to and routine demonstration of biopsychosocial approach to care of individuals and families. + Dedication to delivering a team-based integrated model of care + Demonstrates cultural humility and anti-racism values and works toward ensuring all patients feel welcome. REQUIRED LICENSES AND CERTIFICATIONS + Master's degree from an accredited school of social work.
