The VNA Health Group is seeking a  Medical Social Worker for Essex County with availability to cover additional counties such as Hudson, Union, Passaic and Morris as needed. This position is responsible to  provide psychotherapeutic counseling and support to End of Life care for hospice patients and families, with a particular emphasis on those experiencing psychosocial and/or interpersonal challenges.

Responsibilities Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workersDevelops and implements a social work plan of careCollaborates with professional staff on psychosocial problems and appropriate interventionsProvides patients and families with information on community resource options and assists in linking with needed resourcesAdheres to the organization’s policy and procedure on documentationAdheres to agency infection control and safety policiesAdheres to the organization’s policy in regards to absenteeism and appearanceOmission of specific duties does not exclude them from this position if the work is similar, related or a logical assignment for this position Qualifications Masters Degree in Social Work required with current NJ LSW1+ year of supervised clinical experience; community health experience preferredExcellent verbal and written communication skillsExcellent interpersonal skillsAbility to work with multiethnic and diverse socioeconomic populations

 Working Conditions/Physical Demand: Services are provided in the patient’s home in a community or facility setting
