Social Worker

Position Summary: 

The Social Worker works in collaboration with the integrated health team to meet the individual needs of multiple patients throughout the system. Social workers will identify needs of patients, build rapport, and address patient concerns and help identify goals. They will assess the needs, monitor and evaluate outcomes, and coordinate care between community partners, and across the system. Responsible for providing case coordination and full range of services including assessment, treatment planning, and therapeutic interventions in an interdisciplinary environment consistent with the position’s qualification, professional practices and ethical standards.  Serves as a liaison between the integrated health care team and partner agencies in the community for the purpose of collaboration to reduce utilization, improve access, and address the needs of the community. Advocate for the patient within the health care system and assist the patient in navigation of care across the health care continuum. Participates in interprofessional treatment planning with the other members of the healthcare team, using effective communication, collaboration, and patient-centered care. The incumbent shall also demonstrate accountability for and contribution to program development, quality improvement, problem solving and productivity enhancement in a flexible interdisciplinary fashion. Assists in performance improvement activities within the department and participates in departmental quality initiatives, as well as other appropriate meetings.

Essential Duties:

Performs initial assessments on patients to determine appropriate social service needs. Develops individualized treatment plans; always documents assessments.

Case management, referral, and service coordination

Meets regularly with patients

Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.

Communicates with physicians regarding social service needs of the patient.

Maintains current knowledge of resources available within the community. Maintains supply of resource materials to be distributed to patients when needed.  Is able to obtain other resources as needed.

Collaborates with Case Managers and other members of the healthcare team to provide for psychosocial assistance.

Treats patients and families with respect and dignity.  Identifies and addresses psychosocial needs of patient and family.

Interacts professionally with patient/family and involves patient/family in the formation of the plan of care.

Maintains and respects confidentiality of patient/physician information.

Initiates appropriate referrals to community agencies.

Education and Experience:

Experience in Care Management and interdisciplinary patient care

Master’s degree in relevant field preferred (e.g., Social Work, Counseling, Rehabilitation).

Licensed or licensed-eligible within 12 months (e.g., LSW, LMSW, LCSW, LCPC, LADC).

3 or more years of previous experience in all aspects of social service
