The Grand Rehabilitation and Nursing in Utica is seeking a full time Social Worker for our skilled nursing facility

Duties Include:

Responsible for assessments, care planning & helping residents adjust to their new surroundings. Discharge planning experience preferred Familiar with a variety of the field's concepts, practices & procedures. Relies on extensive experience & judgment to plan and accomplish goals. A wide degree of creativity and latitude is expected Occasional Community Outreach

Requirements Include: Candidates must have:

A current/valid Bachelors Degree. Long-Term Care exp. preferred Exceptional interpersonal & leadership skills Knowledge of applicable state and federal guidelines.
