**Social Worker - Sub Acute Care**
**St. John’s Hospital Camarillo**
**St. John’s Hospital Camarillo** is seeking a compassionate and experienced **Social Worker** to join our dedicated team. We provide high-quality care to patients recovering from illness or injury, helping them regain their independence and return to their communities.
**Responsibilities**
+ Conducting comprehensive psychosocial assessments of patients to identify strengths, needs, and challenges related to their illness, recovery, and discharge planning.
+ Developing and implementing individualized treatment plans that address patients' social, emotional, and psychological needs.
+ Providing counseling and support to patients and their families, addressing issues such as grief, loss, adjustment to disability, and coping mechanisms.
+ Collaborating with physicians, nurses, therapists, and other healthcare professionals to coordinate care and ensure a holistic approach to patient treatment.
+ Facilitating discharge planning, including connecting patients with community resources and support services.
+ Maintaining accurate and timely documentation of patient assessments, interventions, and progress.
+ Advocating for patients' rights and ensuring their needs are met.
+ Participating in case conferences and team meetings.
+ Staying current with best practices in social work and sub-acute care.
**Qualifications**
+ At least one year of clinical and/or medical Social Work experience is required.
+ Master's Degree from a School of Social Work accredited by the Council of Social Work Education required
+ Knowledge of contemporary behavioral and systems theories relevant to health care; end-of-life dynamics and interventions; grief and bereavement counseling; substance abuse identification and interventions; support for victims of abuse, neglect, or violence; community or financial resources for underserved or vulnerable populations.SSDI, SDI, and other governmental/county requirements/programs. In California, a thorough comprehension of OBRA and Title 22 is required.
+ Excellent customer service and presentation skills are a must.
+ Strong interpersonal and written communication skills are essential Demonstrated ability to apply analytical and problem-solving skills Demonstrated ability to manage multiple tasks or projects effectively
+ Ability to work independently with a high degree of detail orientation.
+ Ability to work efficiently in a fast-paced environment with changing priorities.
**This position is represented by SEIU-UHW and is covered by the terms and conditions of the collective bargaining agreement.**
**Pay Range**
$40.23 - $47.82 /hour
