\t\t\t\t\tMonday - Friday 8:30am - 5pmJob Summary:Coordinates patient care services and resources to assure optimal clinical quality and financial outcomesWorks collaboratively with the Case Management Coordinator and other health care team membersUses screening mechanisms to identify patients who are at risk for lengthy inpatient stays, complex clinical issues, high resource use or repeat admissions due to chronic diseases. Intervenes within 24 hours of patient admissions. Documents according to established guidelines.Educational Requirements/Preferences:Bachelor’s Degree in Behavioral Health, Social Work, or related fieldLicensure/Certification/Registration: Maryland Social Work licensure required or LSWA. Current Maryland LCSW preferredExperience/Requirements:Experience as a social worker in a hospital environment preferred

Pay and Benefits
The pay range for this position is $35.00 - $48.00/hr.
