The position works with all populations, except for minors, including homeless individuals and families and those at risk of becoming homeless. Social Workers will work in one of the following units Mental Health, Home and Community Based or Family Support within the Division of Programs of Metro Social Services. This position assists customers with navigating the different social services systems and obtaining necessary information, documents, resources, and case management services to help secure housing and address other social needs for customers. Service areas include but may not be limited to counseling, information, and referral, short and long-term case management, home and community-based services, mental health services and other assistance. Typical Duties All Social Worker duties: + Perform concise, comprehensive social assessments and developing service plans based on the customers' individual needs, assessments of individuals which may include individuals with acute or persistent mental disorders or cooccurring disorders. + Provide information and referral services to customers and the public that enables them to utilize the programs' services to best address their needs. + Responsible to serve customers in the office, the community and homes depending on the individual circumstances. + Provide short-term or long-term case management services to all populations served. + Coordinate services, plan, and maintain ongoing contact with customers for completion of services. + Submit monthly reports and enter data into multiple case management systems and computer applications. + Responsible to be able to provide basic care, counseling, and assistance to individuals with mental illness or developmental disabilities. + Responsible to complete basic social and psychological needs, problems, attitudes, and behavior patterns of individuals with acute or persistent mental disorders or co-occurring disorders. Duties specific to assigned units, Mental Health, Home and Community-based, or Family Support: + Responsible to understand community resources such as public assistance programs, drug rehabilitation programs, and housing shelters. + Responsible for handling wellness checks for customers via telephone and in person. + Identify, arrange and/or coordinate services to assist customers with daily living in their homes. + May provide services to assist customers with daily living in their homes and to better their quality of life. + May conduct interviews, provide written summaries, and perform case management duties for assigned Conservatorship cases. + Build and maintain working relationships with various Metro Departments and other community agencies to ensure the emotional and social well-being of the assigned constituents are being met, assessed, and documented on a regular basis. + May occasionally work Overflow Shelter shifts. + Attend community events virtually or in-person. Minimum Qualifications EDUCATION AND EXPERIENCE Bachelor’s Degree in Social Work from an accredited college or university *No Substitution LICENSES REQUIRED Valid Driver License may be required for some positions in this classification. Preferred Experience, Knowledge, Skills, and Abilities + Two (2) or more years of professional social work or case management experience in the mental health, domestic violence and/or substance abuse field + Experience working with diverse populations including the homeless and individuals with mental health concerns + Skilled in interviewing, making case assessment, counseling, and assisting clients in goal formulation and realizations and case referrals + Experience or knowledge of Domestic Violence, Mental Health, Home and Community Based Services, Homeless, Trauma, Elderly & Disabled, Veterans and the issues + Experience and/or knowledge of the Two-Generation approach, Advanced Childhood Effects (ACES) and Trauma Informed care + Bilingual We are an equal opportunity employer that values diversity at all levels.
