Social Worker
Details Department: Physician Support ServicesSchedule: M-F   730a (8a) to 4p(430p) Hospital: Ascension, remote position with possible travel neededto provide in office support. Location: Westfield, INThe AMG PrimaryCare Provider Navigation Team is a multidisciplinary team made up of LPNs, RNs, and licensed Social Workers that performs case management (CM))  in the post acute environment.The Social Work Navigator collaborates with the RNs and LPNs in the primary care physician offices to work with our patient population on several different Behavioral Health issues like:

Education about their chronic conditions

Financial assistance

Medication assistance

Assistance with Assisted Living or SNF placement

Behavioral Health Issues 

Safety concerns involving endangerment/abuse

Concerns for drug or alcohol abuse

Facilitation of family discussions regarding advanced care planning, safety, and end-of-life care

Caregiver stress and burden to formulate an in-home care plan

Responsibilities

Investigate, assess and plan interventions to help patients cope with social, emotional, economic and environmental problems.

Interview patients, assess priorities and document case activity.Consult with team managers, statutory and voluntary agencies and patient's relatives to ensure compliance with patient's treatment plan.Assist clients/families with obtaining community assistance by referral to proper resources.Develop and maintain working relationships with community health, welfare and social agencies and seek creative means to assist the client with his/her needs.Support management of concurrent review, discharge planning processes, social services, home care and other post discharge needs.
Requirements

Licensure / Certification / Registration:

Licensed Social Worker credentialed from the Indiana Behavioral Health and Human Services Licensing Board obtained within 12 Months (1 year) of hire date or job transfer date required. Or Master of Social Work (LMSW) obtained within 12 months (1 year) of hire date or job transfer date.Drivers License preferred

 

Education:

Bachelor's degree in Social Work required.

 

Work Experience:

3 years of experience preferred.
