Scope of Job
To provide social work services to the Oncology program for the assigned population, supporting care for adult patients and their families. This includes, but not limited to, providing consultation on social, environmental, psychological, financial, and cultural factors related to living with cancer.
Description of Duties
• Works in partnership with the Oncology program leadership and other health care professionals to ensure continuity and coordination of care for adult patients and families. Services include, but are not limited to, consultation to patients/families regarding social, environmental, psychological, financial and cultural factors related to living with cancer, facilitation of groups, family conferences, one-to-one emotional support, supports continuity with the Care Management program.
•Conducts biopsychosocial assessments using appropriate diagnostic criteria to recommend, facilitate, and implement intervention strategies for a comprehensive treatment plan.
•Uses counseling techniques, resource knowledge and linkage throughout the disease trajectory, including end of life, survivorship and bereavement.
•Provide comprehensive biopsychosocial assessment and counseling services to individuals, families and groups. Supports compliance with treatment protocols, advance care planning and assists with resource linkage as a part of a multidisciplinary healthcare team.
•Collaborates with patients, the inter-professional team and patient support systems in treatment planning and supports the patient in transitioning from the clinic to long-term care.
•Provides education on social services and public systems.
•Screens, assesses, and secures background information from patients and their families as part of the clinic new patient intake process.
•Documents information accurately and timely in the electronic medical record.
•Shares information obtained in patient and family interviews with physicians, nursing staff, and other clinic and community personnel as necessary for continuity of treatment.
•Engages in outreach and training activities with hospitals, social service providers, and other community partners.
•Initiates frequent contact with local and state agencies.
•Participates in program evaluation activities, including but not limited to data collection, grant reporting, patient satisfaction surveys, preparation and publication of peer-reviewed manuscripts, and other activities as needed.
•Performs related duties as required.

Direct supervision of all assigned staff.
Required Education
Master’s degree in Social Work ( MSW or MSSW ) from an accredited university in an accredited program approved by the Council of Social Work Education ( CSWE ).
Preferred Education

Licenses/Certifications
• Current Licensed Master Social Worker ( LMSW ) or Licensed Clinical Social Worker ( LCSW ) by the Texas State Board of Social Worker Examiners.
Required Experience
One (1) year of professional social work experience.
Preferred Experience
One (1) year experience in an oncology, hospital or hospice setting.
Preferred: Bilingual
Equipment
Use of standard office equipment.
Working Conditions
Needs to be able to successfully perform all required duties. Usual office environment. Some travel and weekend work may be required. UTRGV is a distributed institution, which may require presence at multiple locations throughout the Rio Grande Valley.
