The Adolescent Health Clinic Social Worker provides a professional continuum of services to address the psychosocial needs of patients and families. The Social Worker partners with the Adolescent Health Providers to support coordination of care and mental health interventions as needed. This trauma-informed care includes acute mental health assessments, crisis stabilization, treatment planning, counseling, and resource referrals. The clinic social worker establishes plans for post hospital discharge, transition and community care follow-up to the most appropriate levels of care. This clinic position has a particular focus on Eating Disorders, Anxiety and Depression and Gender Dysphoria. The Adolescent Health Clinic Social Worker is an identified expert in pediatric mental health issues and provides both consultation and educational programming to OHSU staff and the community. As part of the position, the social worker will actively participate in program development, evaluation efforts, and building relationships with community partners. Specific programmatic goals include providing bridge counseling to patients until they can establish with a long-term provider, improving the quality of care provided to patients and families, increasing the competency of staff in responding to patients with mental health needs, and improving the patient and family experience of care. This position will participate in regular review of screening tool and program development processes for improvements of tools based on data.
This position is a flexible work position, which allows some remote work. Remote work at OHSU is a work arrangement under which an employee performs the duties and responsibilities of their position, and other authorized activities, from a non-OHSU location as part of their approved, regular work schedule. The Social Worker generally is expected to be in person at least half of their work time (but often more), depending on the expectations of their specific position. The social worker should have a private space at home, dedicated to work, and no Protected Health Information should be printed. Social Workers are expected to see patients in person when at all possible, which may require coming to the hospital at short notice on a remote day.
Function/Duties of Position Conducts psychosocial evaluations of patients and families experiencing mental health emergencies for youth and families referred for mental health services.Participates in treatment planning and goal-setting with patients and families. Provide brief mental health counseling and intervention.Participates in coordination of care efforts including determination of the most appropriate level of care and makes indicated resource referrals.Provides education regarding pediatric mental health issues to OHSU staff and community members.Maintains accurate and timely documentation of patient care activities an efficent manner per departmental policies. Participates in staff meetings and QI Projects and and Improvements. Meets in regularly scheduled supervisory sessions. Participates in patient rounds as appropriate. Complete all OHSU/Joint Commission mandatory education requirements and activities within established time lines. Yearly competencies as identified by department Required Qualifications Master’s of Social Work1 year of mental health counseling experience with pediatric patients and families.Psychosocial assessment, treatment planning, counseling, crisis intervention, short term counseling, and advocacy. Excellent task prioritization, time management, and communication skills.Provision of care for patients with Eating Disorders and Gender Dysphoria.
