SUMMARY
Assists physician and health care team members in understanding the significant social and emotional factors related to the health problems; participates in the development of the plan of treatment; prepares clinical and progress notes; works with the family; utilizes appropriate community resources; leads patient groups; and participates in discharge planning. The SW will be assigned to the Behavioral Health Unit and work with both the geriatric and adult populations.
Qualifications include:
ï‚· Bachelor’s Degree in Social Work required, Master’s preferred.
ï‚· License in Social Work required
ï‚· Knowledge regarding community resources required.
ï‚· One year of hospital/health care experience preferred.
ï‚· Excellent communication skills and ability to work in a collaborative team approach required.
