Description
We are seeking a full-time Social Worker for our Brookside School!
Under the supervision of the Director of Educational Services, the School Social Worker will provide individual and small group counseling (play therapy) to young children who receive special education services as approved in their Individualized Education Plan (IEP) as well as case management services to parents of children attending Brookside School to assist them in finding services for their child and family.
Responsibilities include but are not limited to:
+ Maintain all records of services in the county owned monitoring system for preschool and manually for any school age students who receive counseling.
+ Provide Parent Counseling as approved in student's IEP's and also for any family in need of this service and document session notes appropriately.
+ Update IEP's in a timely manner for each student on caseload.
+ Serve as primary liaison to Department of Social Services and other agencies for children receiving services such as Foster Care and Child Protective Services (CPS).
+ Assist staff or take on responsibility to submit any necessary reports to CPS to ensure that all pertinent information is shared to best meet the needs of the student.
+ Schedule and facilitate classroom meetings to ensure communication and share recommendations to meet student needs.
+ Be available to assist families of UPK and other "typical" students as needed.
+ Provide case management services to families including advocacy and referral and assistance in accessing services to assure that students' and families' needs are being met.
HOURS: 40/Monday through Friday - 8:30 am to 4:30 pm
STARTING SALARY: $ 54,661.36 to $ 60,127.50 per year
Minimum Requirements
+ NYS Licensing as LCSW-experience working with young children and families.
+ Ability to work both independently and as part of a multi-disciplinary team.
+ Ability to work collaboratively with parents, school team, school district, counties, and agency administration to effectively implement student programs and facilitate achievement of student goals.
