JOB SUMMARY
The Social Worker - BSW evaluate the impact of psycho-social problems on identified patients in accordance with physician's prescribed plan of treatment.
JOB QUALIFICATIONS
EDUCATION
For positions requiring education beyond a high school diploma or equivalent, educational qualifications must be from an institution whose accreditation is recognized by the Council for Higher Education and Accreditation.
Minimum Required: Bachelor of Social Work Degree from accredited university.
Preferred/Optional: None
EXPERIENCE/KNOWLEDGE/SKILLS/ABILITIES
Minimum Required: One-year social work experience in an acute care hospital or health related agency. Understanding of community health, welfare, and social agencies. Ability to communicate well and work effectively with patients and/or families, physicians, other home health staff, community agencies and public.
Preferred/Optional: Home health experience.
CERTIFICATIONS/LICENSES
The following licensure(s), certification(s), registration(s), etc., are required for this position. Licenses with restrictions are subject to review to determine if restrictions are substantially related to the position.
Minimum Required: Current social work certification by applicable state. Applicable state board of licensing and regulation certification within 30 days from date of hire. BCLS within 30 days from day of hire. Must possess valid driver’s license, automobile insurance and a reliable vehicle.
