Social Worker Care Coordinator – LICSW or LGSW in DC, MD, VA

Standard Hours:  Monday - Friday normal daytime business hours (no on-call, no weekends and no holidays required). 

This is a field based position in the greater Washington DC area and you will be in the field 100% of the time going into members homes.   

This position is open to candidates who live in DC, MD, or VA

This is a field-based position visiting our members in their homes, community-based housing, and long-term, skilled and acute care facilities in the Washington DC area. You'll need to be flexible, adaptable and, above all, patient in all types of situations.

 

Primary Responsibilities:

Assess, plan and implement care strategies that are individualized by patient and directed toward the most appropriate, lease restrictive level of care Identify, initiate, and follow up on referrals for community based programs and providers; including financial, psychosocial, community and state supportive services Manage the care plan throughout the continuum of care as a single point of contact Communicate with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously to all members Advocate for patients and families as needed to ensure the patient's needs and choices are fully represented and supported by the health care team

Required Qualifications:

Master’s degree in social work or other related clinical field Active and unrestricted LICSW or LGSW license in Washington D.C. or ability to obtain Washington, D.C. License within 90 days of hire 2+ years of experience in long-term care, home health, hospice, public health or assisted living 2+ years of experience working with MS Word, Excel and Outlook 1+ years of clinical or case management experience Driver’s License and access to reliable transportation Ability to work in a field-based capacity in Washington, D.C. Reside within 50 miles of Washington, D.C.

 

Preferred Qualifications:

Experience with electronic charting Experience with arranging community resources Experience working with elderly/geriatric patients Field-based work experience Background in managing populations with complex medical or behavioral needs

 

 

Maryland and Washington, D.C. Residents Only: The salary range for this role is $58,300 to $114,300 annually. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
