VillageCare is looking for a self-motivated and passionate Social Worker Care Manager, who will provide telephonic assessments of member's status and needs. This position works with our patients to develop, implement, and monitor their care plan and ensure continuity of care while monitoring long term care needs. Some daily activities will include:\n\nProviding all aspects of care management such as planning, coordination, assessment of health, environmental, and psychosocial status of the members on an ongoing basis.\nIdentify health, environmental, and psychosocial risks for assigned members and recommend appropriate interventions.\nProvide an on-going assessment of the effectiveness of the Member's plan of care.\nAssist in discharge planning of member from inpatient setting.\nGather information to assist with utilization review decisions, using clinical criteria as a guide for data collection.\nMake requests for authorization to the Care Management Supervisor for new services and changes to the plan of care.\n Assist, as needed, in the semi-annual reassessments using the New York State designated assessment tool and other plan defined assessment tools.\n\n

We would like to speak to those who Minimum formal education of Master's Degree and a current NYS LMSW license. Minimum of one (1) year job-related experience in MLTC. Bilingual skills highly preferred. \n
