Job Summary and Responsibilities Acentra Health is hiring Social Worker Case Manager to join our growing and dynamic team! As a Social Worker Case Manager, you will play a critical role in supporting and advocating for the well-being of those we serve. In this role, you will assess beneficiaries’ needs, develop personalized care plans, and coordinate with healthcare teams to ensure the best possible outcomes. This is a community-based role that requires travel throughout the state of Maine to assist with care coordination and ensure beneficiaries receive the appropriate support and services.This hybrid role offers the ideal balance of flexibility and purpose—work from the comfort of your home office while also engaging directly with patients in the field. Travel locally with expenses covered, including mileage and parking reimbursement Job Responsibilities: Develop and implement individualized care plans in collaboration with beneficiaries to ensure continuity of care. Apply clinical expertise, strong communication, problem-solving, and conflict resolution skills to achieve optimal beneficiary outcomes while adhering to client requirements. Foster shared accountability between beneficiaries, caregivers, and providers, ensuring active participation in care planning. Coordinate care by working closely with beneficiaries, healthcare providers, and the broader healthcare team to deliver comprehensive support. Identify and address gaps in care and compliance to enhance patient outcomes and well-being. Uphold strict confidentiality standards, ensuring the protection of all client-related information. Adhere to organizational, state, and federal confidentiality regulations and policies to maintain compliance and ethical practice. Manage caseloads efficiently, ensuring timely and accurate documentation and entry of assessment results into the web-based database in accordance with regulatory requirements and company policies. Maintain open and effective communication with all relevant stakeholders to ensure seamless care coordination. Engage in ongoing education, certification, and professional development to stay current with best practices in case management. Qualifications Required Qualifications, Skills, Knowledge and Experience Bachelor’s degree in Social Work (BSW) required; Master of Social Work (MSW) or Case Management Certification (CCM) preferred. Active, unrestricted Social Worker license required. Minimum of 3 years of social work experience specializing in older adult populations within a healthcare setting. Required: 3 to 5 years of clinical experience, including at least 1 to 3 years of case management review experience. Experience working with commercial health plans is preferred. Proficiency in Microsoft Office Suite, including Word and Excel. Ability to work telephonically and navigate computer systems efficiently. Excellent written and verbal communication skills with the ability to build and maintain professional relationships. Strong problem-solving skills, with the ability to analyze situations, identify solutions, and implement appropriate actions. Ability to multi-task and prioritize responsibilities under variable and sometimes conflicting deadlines. Superior attention to detail and demonstrated ability in decision-making. Flexibility and strong organizational skills, ensuring effective case management. Maintain confidentiality and adhere to all privacy regulations. Strong understanding of cultural competency, ensuring sensitivity when working with diverse populations. Demonstrated initiative and sound judgment in performing job responsibilities. Maintain professionalism, flexibility, and dependability under pressure. Ability to work independently and collaboratively as part of a multidisciplinary team. Strong customer service orientation, including the ability to engage with internal and external stakeholders effectively. Assist and conduct outreach to members, particularly those who have not engaged previously. Ability to research, identify, and apply appropriate standards of care for case management. Compensation The pay for this position is $30.00 to $32.00 per hour Based on our compensation philosophy, an applicant’s position placement in the pay range will depend on various considerations, such as years of applicable experience and skill level. #LI-JS1 Pay Range USD $63,360.00 - USD $99,000.00 /Yr.
