36 hours per week- Night Shift 7:00pm-7:30am- rotating weekends & holidays

GENERAL SUMMARY:

Coordinates the overall interdisciplinary plan of care for patient, from admission to discharge.

Responsibilities:

Creates plan for care across the continuum, integrating patient/family preferences and values. Serves as a resource to assess, evaluate, recommend and treat legal, neglect, substance abuse, crisis, psychosocial and various adjustment needs.| Advocates for resources and removal of barriers. Maintains ongoing dialog with supervisor and care transition team members to ensure effective implementation and reevaluation of health plan. Acts as a resource for adequate medical record documentation, complex level of care recommendations, and services as they relate to diagnoses, and treatment options for post-discharge care.

EDUCATION/EXPERIENCE REQUIRED

Master's Degree in Social Work required.  Current licensure by the Michigan State Social Worker required Previous Case Management experience preferred LLMSW or LMSW with 2 years of inpatient or ED Behavioral Health Experience Additional Information Organization: Henry Ford Health St. John Hospital Department: Social Service 001 Shift: Night Job Union Code: Not Applicable
