Performs psychosocial assessment, counseling and referral of patients; provides a variety of casework and social services to patients and their families; provides patient with insurance information in accordance with Courtland Dialysis Policies, Procedures and Guidelines, OSHA, HCFA, AAMI, federal, state and local regulations.
Responsibilities
The following duties and responsibilities generally reflect the expectations of this position but are not intended to be all-inclusive.
Provides support and direction for patients and families through casework including interviews, counseling, conferences, crisis intervention and group work to facilitate optimal social and mental health for patients.Maintains documentation per Chesapeake Dialysis policy, procedures, guidelines, federal, state and local regulations.Participates in patient care conferences; collaborates with other members of multidisciplinary team to develop comprehensive evaluations or plans relevant to appropriate interventions.Maintains liaison with community agencies; participates in area activities related to health and welfare services; serves as an advocate for the patient with local, state and federal agencies and their respective programs.Provides information to patients regarding available transportation. Participates in appropriate research activities approved by Chesapeake Dialysis.Participates with multidisciplinary team to facilitate patient’s adherence to their plans of care.Promotes a constructive approach to problem solving within the facility and takes an active role in QA/CQI process.Utilizes capabilities of the computer systems.Educates and counsels patients regarding Chesapeake Dialysis Directives and provides follow up per policy for Advance Medical Directives.Coordinates social work in-service education for health professionals. May provide in-service education for community organizations if requested with the approval of the Director of Nursing.Participates in and promotes team concept throughout all clinical activities in accordance with policies and as allowed by state regulations. Qualifications
Master of Social Work with specialization in clinical practice from a college or university accredited by the Council of Social Work Education; and must be a licensed or certified Social Worker as required by individual state law. Prefer at least one year experience and knowledge of ESRD.
