Social Worker (BSW) – Home Health/Hospice provides psychosocial support for patients, collaboratively working toward program objectives of achieving the best possible quality of life for patient and family. The Social Worker (BSW) - Home Health/Hospice provides services and care in accordance with a comprehensive patient care plan and under the supervision of a Social Worker (MSW) – Home Health/Hospice **JOB QUALIFICATIONS** **EDUCATION** _For positions requiring education beyond a high school diploma or equivalent, educational qualifications must be from an institution whose accreditation is recognized by the Council for Higher Education and Accreditation._ **Minimum Required:** Bachelor’s degree in social work or related field. **Preferred/Optional:** Master’s degree in social work. **EXPERIENCE** **Minimum Required:** One year of social work experience in a healthcare setting. **Preferred/Optional:** Three years’ experience working as a Social Worker working with a target population, community health setting or in a healthcare facility working with children and adults. **CERTIFICATIONS/LICENSES** _The following licensure(s), certification(s), registration(s), etc., are required for this position.
