The social worker is responsible for the implementation of standards of care for social work services in the home health and hospice setting.
Essential Job Functions/Responsibilities
Assesses the psychosocial status of clients related to the client’s illness and environment and communicates findings to the registered nurse.
Carries out social evaluations and plans intervention based on evaluation findings.
Maintains clinical records on all clients referred to social work.
Provides information and referral services to clients and families/caregivers regarding environmental needs and
Provides information to clients or families/caregivers and community agencies.
Markets and promotes the agency programs and events in the local community.
Serves as liaison between clients or families/caregivers and community agencies.
Maintains collaborative relationships with organization personnel to support client care.
Maintains and develops contracts with public and private agencies as resources for client and organization personnel.
Participates in the development of the total plan of care and case conferences as required.
Assists medical team in understanding significant social and emotional factors related to health problems.
Participates in discharge planning.
Coordinates new volunteer training, continued education, and annual evaluations.
Assigns volunteers to each hospice family and coordinates visits.
Coordinates the bereavement program.
In conjunction with the agency director, the social worker will be the contact person with the foundation department at the local hospital.
Responsible for event coordination, planning and execution.
Assures proper documentation of social services in clinical records in compliance with state, federal, and Joint Commission standards.
Other duties as delegated by the Clinical Director/Supervisor.
Qualifications
Bachelor's degree in Social Work
Licensed as a Social Worker in Minnesota and North Dakota
One year experience in healthcare
Excellent verbal, written communication skills and organizational skills
Basic computer skills
Preferred
Master’s degree in Social Work - salary dependent on experience
Experience in Home Health and Hospice
