Position Summary 

Come join the small but mighty team at North Shore at Home - Hospice, a new up-and-coming hospice agency in the Milwaukee and Central WI area. Our hospice agency is seeking a Medical Social Worker. Hours are flexible and can be adjusted to fit your schedule between 8-5 PM, Monday-Friday. The ideal candidate would also be able to pick up shifts in an after hours and weekends on-call rotation. Mileage reimbursement is provided.

Job Duties:

Supervised by Lead Social Worker and reports to Director of Clinical Services.

Provides visits to patients and families following a regular frequency, and as needed.

Completes psychosocial assessments on patients and families.

Completes legal documents with new hospice patients.

Assesses changes to psychosocial and emotional factors related to the patient’s illness, need for care, response to treatment and adjustment to care.

Assesses the relationship of the patient’s medical condition to the individuals home situation, financial and legal resources, and availability of community resources.

Makes referrals to community resources as needed.

Provides emotional support and education to patient and family.

Participates and cooperates with interdisciplinary care team members and attends IDG meetings as needed.

Qualifications:

Holds a Master’s degree in Social work and an active license in the state of Wisconsin.

One year of medical social work experience preferred, but not required.

Valid driver’s license and vehicle insurance.
