Position Summary:
Assesses, intervenes, and reassesses the social and emotional factors for clients and their families/caregivers during end of life. Participates as an active member of the Hospice Interdisciplinary Team (IDT). Assists clients and their caregivers in understanding and coping with the emotional stress of terminal illness and provides supportive education and resources to aid them in preparation for end of life. Provides education, resources, and referrals in the event of live discharges. Actively participates in bereavement activities.

Master’s degree in Medical Social Work Active NYS Medical Social Work License (LMSW) Valid Driver's licenseExperience:
Minimum two years post graduate experience as a social worker. Previous home care or palliative/hospice experience preferred. Dependable transportation is required.Essential Functions:
Provides “home” visits- which includes client homes, nursing homes and hospitals- to assess patients and their support system (family, friends) psychological, emotional, financial, and social needs. Provides end of life supportive counseling activities for all clients including the client’s support system. Documents all client assessments, interventions, and communications in EMR in a clear and timely manner. Actively participates in appropriate committees for enhancing Hospice services. Works with clients, their support systems, and the Hospice IDT to assist in resolving problems related to the individual’s environment, level of care, financial resources, community resource needs, pre-grieving counseling, funeral home arrangements. Prepares reports, referrals, communication, and continuous quality improvement data. Regularly attends the Hospice IDT meetings and Hospice case conferences assisting staff members in understanding and planning for social and emotional factors in the development and updates to the client’s plan of care. Provides in-service programs to hospice staff, volunteers, and community agencies. Assists with the design and implementation of client education and Hospice activities including but not limited to bereavement, We Honor Veterans, marketing, community education. Regulations, Laws, and GuidelinesMaintains and implements rules, regulations, policies, procedures, laws, and guidelines including Joint Commission, that impact the Social Work field.
Other Duties:
1. Local travel for this position is required.
2. Participation in community and employee engagement activities is required.

Education, License & Certification: Master’s in social work and Licensed Medical Social Worker in New York State. Valid driver’s license.
The pay range for this position is $30.34-$48.07
