Social Worker II, FT McLeod Oncology & Hematology Seacoast

Founded in 1906, McLeod Health is a locally owned and managed, not for profit organization supported by the strength of more than 900 members on its medical staff and more than 2,900 licensed nurses. McLeod Health is also composed of approximately 15,000 team members and more than 90 physician practices throughout its 18-county service area. With seven hospitals, McLeod Health operates three Health and Fitness Centers, a Sports Medicine and Outpatient Rehabilitation Center, Hospice and Home Health Services. The system currently has 988 licensed beds, including Hospice and Behavioral Health. The hospitals within McLeod Health include: McLeod Regional Medical Center, McLeod Health Dillon, McLeod Health Loris, McLeod Health Seacoast, McLeod Health Cheraw, McLeod Health Clarendon and McLeod Behavioral Health.  

Essential Job Responsibilities/Expectations

Job responsibilities include those listed in competency document. Maintains a professional image and exhibits excellent customer relations to patients, visitors, physicians, and co- workers in accordance with our Service Excellence Standards and Core Values. Provides psychosocial services to patients, families, and caregivers facing the impact of a cancer diagnosis and treatment through all phases of the cancer continuum of care. Facilitates adjustments of patients and families to a cancer diagnosis, treatment and survivorship by promoting coping skills, self-advocacy, sharing information and resource referrals. Operates from a self-help perspective to maximize optimal patient functioning, navigating health care resources to reduce distress and improve survivorship. Serves as a patient advocate and liaison between the patient and medical team. Adheres to national professional social work code of ethics and standards of care as endorsed by the National Association of Social Work and Association of Oncology Social Work. Assesses financial needs of new and established oncology patients related to clinical care, to aid in reducing financial barriers and provide counsel and case management for those requiring services. Educates patients regarding insurance optimization opportunities and educates qualifying patients regarding entitlement programs such as SSI, SSDI, Medicare Savings Program, Low Income Subsidy, and Medicaid. Maintain patient financial education materials, collect data, run statistics and monitor productivity. Assist with tasks to improve processes to facilitate communications and service between patient, clinic and care giver teams to ensure accurate and timely delivery of all cancer related treatment. Discuss Potential needs the patient may have that would present barrier to care. Interacts with each individual patient, preferably fce to face, new to MRMC chemotherapy and/or radiation therapy. Will also educate each patient about HOPE resources available to the patients and their families. Triages patients as appropriate to additional human resources as needed ie: patient Oncology navigator, Chairty Care Coordinator For Services obviously eligible for HOPE resource assistance makes these resources available to the patient ie: gas cards for transportation, medication assistance, tube feedings through pharmacy, immediate needs as appropriate and other resources as appropriate. Maintains record of all HOPE assistance provided to patients and tracks via spread sheet in real time Reports to Director any potential shortages f needs in order to shift funds within the block grant items. Provides monthly HOPE funds reports to Director and VP Creates and presents quarterly and or as needed reports to HOPE committee

Job Requirements

Licenses/Certifications/Registrations/Education:

In keeping w/ SC LLR requirements for licensure, minimum of a Master’s Degree in Social Work (BSW) from an accredited school. Current South Carolina Social Work License Certified in Oncology Social Work preferred or obtained within 12 months Experience with Medicare health plans, COBRA regulations, Medicaid guidelines, PAP and co-pay assistance programs preferred. ACCC (Association of Community Cancer Centers) Boot Camp (within 1 year of employment)
