As a vital member of our interdisciplinary Home Health and Hospice team, the Social Worker II plays a crucial role in facilitating optimal patient outcomes through a collaborative approach. This position involves assessment, planning, facilitation, and advocacy to address individual health needs effectively. The Social Worker will ensure the efficient and appropriate use of resources across the continuum of care while promoting evidence-based practices and compliance with professional standards.

Full-time | 72 hours every two weeks Monday - Friday | 8:00 am – 5:00 pm No weekends or holidays!Pay and Benefits

Master’s Degree in Social Work Current Licensed Social Worker (LSW) in Minnesota Reliable transportation with a valid Minnesota driver’s license, a driving record in good standing, and insurance liability coverage Familiar with laws regarding confidentiality, vulnerable adult and patient’s rights Minimum of 1 year of home care and/or hospice social work experience preferredTo be eligible for the hiring bonus and relocation assistance, you must be an external candidate who has not worked for CentraCare for at least 9 months and be hired into an eligible full-time or part-time benefited position. Relocation assistance also requires that you currently live more than 50 miles from your primary work location and move within 30 miles of it within 120 days of employment. CentraCare pay promotions to include but not limited to, hiring bonus and relocation assistance may be discontinued at any time due to business needs.
