Position Summary
Provides Clinical Social Work Services in accordance with acceptable Social Work Standards and hospital policies. Provides consultation and direction of Social Services to patients admitted to the Psychiatric Unit, their significant others and patients in the Partial and Intensive Outpatient Programs. Develops, organizes and implements programs, policies and procedures; conducts case work services and counseling; recommends functions necessary to ensure overall operational viability.
Minimum Qualifications
Work Experience
Previous healthcare experience preferred.
Education Masters Degree in Social Work required.
Certifications/Licensure Licensed Clinical Social Worker (LCSW) required. Job Specific Performance Standards The duties listed below are a summary of the major essential functions of this position. Completes comprehensive social history of assigned patient population. Develops and implements treatment plan. Consults with other departments and interdisciplinary team as appropriate to collaborate in patient care and performance improvement activities. Involves patient/family in the formation of the plan of care. Coordinates and directs Social Services to ensure patients’ needs are met and facility policy is followed. Assures that social service/discharge planning activities are integrated and coordinated within the facility by effective communication. Initiates, documents and coordinates discharge planning. Initiates referrals to appropriate community resources. Provides Individual, Group and Family Therapy to patients and families in accordance with the treatment plan. Coordinates family education and involvement in treatment. Participates in interdisciplinary team meetings and shares relevant clinical data on the status of patients. Acts as a liaison to community agencies and significant others with respect to patients progress. Completes documentation in time frame set by policy. Documentation is concise, comprehensive and reflective of services provided. Develops and maintains positive relationships with external community resources and internal hospital departments. Provides social services consultation and intervention services to hospital departments and to external sources. Provides assessment, screening and referral services to psychiatric patients who present to the Emergency Department and to community agencies who initiate telephone referrals. Assists in intake assessments for outpatient programs as needed. Serves as resource for Patient Registration by obtaining clinical information on new admissions for purpose of pre-authorization. In collaboration with other clinical staff, develops and implements new social services programs and maintains established program. Participates in marketing and public relations efforts for outpatient programs through internal and external activities.
