The Social Workers (LCSW or LMFT) evaluates and assesses patient needs for social service assistance, planning and coordination of clinical programs and acts as a liaison between the patient, hospital, Agency, Physician, Insurance Plans and community with in the St. Joseph Home Care Service Lines. Embraces and utilizes evidence based assessment and patient education tools. Understands and promotes the St. Joseph Home Health Social Service Program.

Required Qualifications:

Master's Degree in Social Work. Upon hire: California Clinical Social Worker License or Upon hire: California Marriage and Family Therapist License 1 year Social work experience.

Preferred Qualifications:

Experience in home health care setting.
