The Inpatient Licensed Clinical Social Worker (LCSW) Care Manager provides professional, comprehensive patient centric care management services for at risk patients in an acute care environment. Responsibilities include assessment and planning, coordination of care, discharge planning, documentation of interventions, regulatory compliance and patient advocacy. The goal of the Inpatient Care Management LCSW is to ensure continuity of care for vulnerable patients by identifying and addressing social, financial, cognitive/behavioral or legal barriers to accessing necessary resources and care.
