Job Overview:
The Hemophilia Treatment Center Social Worker assists patients and their families to obtain the maximum of available health care services through psychosocial assessment, counseling, education and referral services. Responsible for the delivery of social work services for pediatric and adult patients and their families diagnosed with coagulation disorders.
Qualifications:
Required:
Graduation from an accredited graduate school of social work with an MSW master's in social work degree.State licensure required by the NJ State board of social work examiners.Licensed clinical social worker LCSW or licensed social worker LSW.One- or two-years experience in a hospital setting preferred.Successful completion of all Orientation ProgramsPreferred:

Provides direct clinical social work services to patients and their families in an interdisciplinary team approachProvide psychosocial assessment and care plan for patients and familiesIdentify barriers to care and develop strategiesProvide education for patients and families to ensure adequate age-appropriate knowledge of coagulation disorders, complications and treatmentMaintains updated education library and educational resources for patients/familiesCoordinate and conduct school visits with patients, parent and hemophilia center teamAct as a liaison to school and child study team for psychosocial and educational issues relating to patients bleeding disorderDelivery of Support Services, individual, couple and family counselingOrganize and facilitate support and education groupsParticipate in weekly interdisciplinary clinic and team meetingProvide expertise on psychosocial aspects of coagulation disorders to hemophilia teamCoordinates community resources necessary to maintaining the continuity of quality medical care by building and maintaining relationships with internal/external resources, making referrals to community agencies and entitlement programs.Understands of all stages of growth and development to assess each patient s age specific needs to provide individual treatment and careEvaluates patient's medical, psychosocial and social condition with respect to their current needs to establish appropriate post-hospital care plans and any services that may be necessary while hospitalized for patients diagnosed with inherited bleeding and clotting disordersFollows documentation policy of social work departmentBenefits and Perks:
