Social Worker-LMSW Position Summary

The Social Worker LMSW plays a pivotal role with the Mobile Integrated Health team of the Community Wellness Department in Population Health Management. This position provides delivers therapeutic services to patients as part of the MIH program. This position is responsible for delivery mental health assessments, crisis intervention, care coordination, and linkage to community resources with a focus on enhancing patient outcomes through integrated care. The Social Worker LMSW will work collaboratively with healthcare providers, first responders, and other team members to deliver patient-centered, culturally competent care in non-traditional settings including patients’ homes, shelters, and other community locations.

 

Social Worker-LMSW Position Requirements  

Education

Master's degree in Social Work or related field required. LMSW required. LCSW-C/LCPC licensure preferred.

Experience

Post-graduate related work experience preferred.

 

Social Worker-LMSW Schedule 

Primarily day shift, Monday through Friday. May rotate weekend hours and work overtime if needs of patients situation require.

 

Social Worker-LMSW Benefits 

At TidalHealth, team members working at least 36 hours per pay period based on 12-hour shift schedules or at least 37.5 hours for non 12-hour shift schedules and part-time team members working at least 30 hours or more on weekends only are eligible for benefits.
