**Community Based Social Worker - West Unity** **Compensation:** $52,000 - $55,000 + Performance-Based Bonus Sign-on Bonus: Eligible Be a part of our growing team in West Unity, OH! An Ohio professional license required. **Working at NYAP** + Phone reimbursement: $50 per month when using your personal device. + 22 Days of Paid Time Off + 11 Paid Holidays + Half-Day Fridays during the summer + Student Loan supplemental Payment assistance after 30 days of employment for qualifying loans + Free supervision for those employees seeking Independent Licensure hours **Responsibilities** • Provides appropriate individual, group, and family mental health services, and other evidence based clinical services as required. • Travels daily, to provide community based services to, and on behalf of, youth and families in compliance with organizational, contract, and regulatory requirements. • Coordinates and monitors clinical services for the youth and family in the community, including mental health, special educational services, medication management, and psychological services. • Regularly reviews and modifies youth-based plans. • Establishes and maintains strong relationship with foster parents and families of origin. • Assists in family reunification and stabilization efforts with the youth’s primary family or other permanent living arrangement consistent with the youth’s case plan. • Provides safety assessment, crisis response, and behavioral stabilization services as required. • On-call rotation duties as assigned. **Minimum Qualifications** + Bachelor's Degree in Social Work (BSW) required + LSW required + Minimum 3 years of solid clinical experience required + Must demonstrate ability to work independently + Strong experience in providing individual and family services required + Experience working in direct service with youth and families required + Experience with TF-CBT preferred + Daily travel required with the willingness to meet clients on a flexible schedule + Must be comfortable providing services in the community and in client homes **Driving and Vehicle Requirements** + Valid driver’s license + Reliable personal transportation + Good driving record + Minimum automobile insurance coverage of $100,000/$300,000 bodily injury liability _NYAP is participating agency in the Great Minds Fellowship and Great Minds Workforce Commitment Incentive Programs._ Apply today! www.nyap.org/employment _Benefits listed are for eligible employees as outlined by our benefit policy._ **Qualifications** **An Equal Opportunity Employer, including disability/veterans.** An Equal Opportunity Employer, including disability/veterans.
