Position Summary:
Assist the Director of Care Management Services in the planning, implementation and evaluation of the care management and case management programs and processes in the hospital. Responsible for the clinical supervision of social workers and direct supervision for staff as assigned within the case management department at UNM Hospitals. Jointly responsible with the Director of Care Management for developing care management processes that are in compliance with federal and state regulations. Assists in developing policies and procedures and assist the Director of Care Management with recruitment and retention, training and quality assurance. Ensure adherence to Hospitals and departmental policies and procedures. Patient care assignment may include neonate, pediatric, adolescent, adult, and geriatric age groups.

Detailed responsibilities:
* SUPERVISION - Provide Clinical Supervision to Social Work Case Managers, conduct random chart reviews and ensure appropriate distribution of assignments
* SUPERVISION - Oversee the work of department personnel by training, allocating and monitoring work; evaluate performance and make recommendations for personnel action; oversees staff hours to best suit departmental and organizational needs
* LEADERSHIP - Provide leadership through identification of problems, opportunities for improvement, action planning, and implementation. Make personnel decisions to include interviewing, hiring, supervising, training, evaluating, disciplining and terminating department personnel
* COLLABORATE - Collaborate with medical staff and other hospital personnel to plan, develop, implement and evaluate protocols, patient care policies and procedures and standards of care
* LIAISON - Serve as UNMHSC liaison to state protection agencies for all reports and/or concerns of patient abuse, neglect or exploitation, and department liaison to the organization's Ethics Committee and Risk Management/Compliance Departments
* LEGAL COORDINATION - Coordinate with UNMHSC Office of Legal Counsel regarding matters involving treatment guardianship and/or other legal issues
* QUALITY - Participate in the quality improvement process including planning, data collection, data review and implementation of resulting recommendations. Deliver quality service and act in a positive way with all customers to complete assigned functions; identify, recommend and implement continuous procedure improvement including meeting the state and federal regulations for discharge planning and utilization management
* PROCESS IMPROVEMENT - Identify, develop and facilitate conceptual and detailed designs to improve departmental and organization efficiency and effectiveness
* COMMUNICATION - Use effective communication strategies with patients, families, physicians, nurses, allied professionals and others to achieve desired patient outcomes and patient satisfaction
* RELATIONS - Develop and maintain positive relationships with community agencies and third party payor sources
* COLLABORATION - Develop collaborative relationships with other departments/services and community health care agencies facilitating and supporting quality care in area of clinical expertise; act as a resource on complex patient care activities
* INTERVENTIONS - Monitor and evaluate short-term and long-term patient responses to interventions in collaboration with quality assurance and utilization review, maintaining interdependent follow-up as necessary
* MEETINGS AND PROFESSIONAL DEVELOPMENT - Attend and participate in professional meetings and committees; stay abreast of new trends, innovations and regulatory oversight requirements
* EDUCATION - Contribute to the education of physicians, nurses, allied health professions and social work interns, and serve as a liaison with educational institutions
* EDUCATION - Assist in identifying educational needs for social work staff but not limited to including needs identified for peer support, new resources, programs and quality issues
* PATIENT SAFETY 1 - Follow patient safety-related policies, procedures and protocols
* PATIENT SAFETY 2 - Demonstrate proactive approach to patient safety by seeking opportunities to improve patient safety through questioning of current policies and processes
* PATIENT SAFETY 3 - Identify and report/correct environmental conditions and/or situations that may put a patient at undue risk
* PATIENT SAFETY 4 - Report potential or actual patient safety concerns, medical errors and/or near misses in a timely manner
* PATIENT SAFETY 5 - Encourage patients to actively participate in their own care by asking questions and reporting treatment or situations that they don't understand or may "not seem right"
* RESOURCE - Acts as resource to professional colleagues; provide staff training, development and consultation as appropriate

* Licensed Clinical Social Worker in New Mexico
* CPR for Healthcare/BLS Prov or Prof Rescuers w/in 30 days
* Cert NM Board of SW Approved Clin Supervisor w/in 18 mos

Physical Conditions:
