Job Overview:
The Social Worker collaborates with members of the healthcare team to facilitate the progression of the patient's treatment and discharge plan.
This position incorporates assessments, care coordination, and education of the patient/caregiver/healthcare team to reach appropriate goals from hospitalization and post-hospitalization care.
Qualifications:
Required:
Master's degree in Social Work (M.S.W.) from an accredited School of Social WorkPreferred:
At least two years of Social Work experience in a healthcare related settingCertifications and Licenses Required:
New Jersey State Social Work Licensure (L.S.W. or L.C.S.W.)Scheduling Requirements:
Shift working, 8:30-4:30pm each dayMonday-Friday and some SaturdaysPer DiemEssential Functions:
Completes initial SW assessment on cases referred in electronic medical recordsInterviews patients/families to obtain psychosocial information relevant to medical problems and for discharge planningInterprets psychosocial situations in relationship to medical condition/hospitalizationCounsels patients/families in regard to adjustment to illness and developing short and long-term goalsCoordinates individual/family conferences in relationship to developing discharge plans and supportive counseling during hospitalizationCommunicates and coordinates with the multidisciplinary team for the purpose of developing discharge plans during team meetingsCommunicates with CM on casesCommunicates and educates the team on the bio psychosocial needs of patients to accomplish the goals and objectives of the hospital for effective patient careProvides ongoing written documentation in patient s electronic record of social work activityDemonstrates knowledge of the growth/development needs of patients/families in the geriatric and adult age levels as reflected in daily activities/responses to patient needsCoordinates/schedules transportationProvides linkage to community agencies for patients/families for specific serviceProvides information/referral for outpatients in ER and other disciplinesProvides education, information and referrals to various community agencies for servicesCompletes Level 1 PASRR screen, EARC-PASProvides information/education on advance directive and POLSTDaily Rounding on specific medical units for discharge planningSubmits data for CCDAProvides education to patient/family on insurance coverage as it relates to hospitalization and discharge planningDemonstrates knowledge of cultural competency in working with patient/families and team membersAdvocates for patients when working with insurance companies and other agenciesMonitors for abuse/neglect for all patient populationsProvides supervision to staff in complex cases requiring a greater degree of professional knowledgeProvides supervision to beginning level social workers and students in the social work departmentProvides assistance to department manager in developing programs for other hospital personnel through formal/informal instructionDevelops and collects data on quality monitoring activityProvides supervision and training for high school students in specific programsCross trained for other hospital areasBenefits and Perks:
