Location: West Chester Hospital Department: Social Work Hours: 8:30am-4pm Shift: PRN- 3 shifts minimum in a 6 week period, with at least 2 shifts being on the weekends, holiday rotation Social Worker (MSW), PRN, First Shift UC Health is hiring a full-time Social Worker for the Social Work's department for the first shift at West Chester Hospital. Social Workers serving in a variety of departments and clinics are responsible for responding to referrals from the interdisciplinary team. The social worker will complete psychosocial assessments, develop and implement a plan of care that addresses patients' needs that promotes continuity of care, and facilitates transitions to the next level of care. The social worker will provide education, consultation, and serve as a liaison to the healthcare team regarding the patient and families' emotional, psychological, social, financial and developmental aspects of care.
