Job Overview:
In an out-patient setting, the Pediatric Social Worker is responsible for assisting in the social case management of 55 current Transplant Patients, Dialysis patients and 30 recent transplant patients. This position will provide discharge planning, referrals to community agencies and entitlement programs. The candidate will also assist with Pediatric Transplant evaluations. Will be responsible for support groups and office events for CKC patients/families. A licensed SW is required for this position.

Participate in family/ patient events organized by the officePerform quality of life assessments for ESRD/ transplant patients + delineate measures required after evaluation donePlan teen events for CKD patients and eventually, small group parent support for those with children with severe kidney disease Provide a process of patient care that includes assessment, diagnosis, treatment planning, intervention, and evaluation of outcomes related to the presenting problemEngage with other professionals to assess, plan, deliver, integrate, and evaluate careSupport, deliver, and/or coordinate appropriate level of care based on patient acuityEducate patients and families about treatment plan, interventions, medications, mental illness, symptom relief, comorbidities, and strategies to enhance overall healthComplete written evaluations, medical records communications, progress notes and service coding utilizing EMR systemProvide brief psychotherapy utilizing principles of CBT, DBT, psychodynamic and/or supportive therapy as neededMeasure progress and effectiveness of treatment and adjusts care plan as neededMaintain confidentiality of patients treatment and records in accordance with HIPPA Other duties as assignedBenefits and Perks:
