Purpose of the Position:
Provide skilled assessment and intervention for children and adults with chronic, complex and/or life-threatening conditions and their families. Provide clinical consultation as active member of interdisciplinary care team. Attend to system dynamics of the health care delivery network to foster best use of resources, effective service delivery, and patient and family satisfaction. Ensure compliance with practice standards and codes. Advocate for interventions that alleviate burden and facilitate coping for PPC families.Job Responsibilities:
Perform thorough psychosocial assessment from a patient and family-centered care perspective. Arrange referrals to appropriate community services and resources. Provide anticipatory guidance for coping with the disease process from a patient and family-centered perspective. Perform comprehensive and ongoing assessment of child/ adult and family coping, and family system integrity. Collaborate with patient and family and team to identify options, goals and priorities for care: advance care planning. Offer patient/ family counseling, including cognitive behavioral skills training to facilitate effective coping. Perform mental health assessment and intervention as relates to coping with the child’s health condition. Facilitate communication among family and team members. Negotiate areas of service overlap and gaps with other (e.g.: inpatient unit) SW staff. Participate in conflict management, bereavement care planning and support, and training and education. Participate in quality improvement activities as assigned or indicated.
