Social Worker (MSW)
**Employment Type:** Part time **Shift:** **Description:** **ONSITE, Full Time** **Position Purpose:** As a Social Worker in the PACE (Program of All-Inclusive Care for the Elderly) program, you will plan, organize, and implement social services for participants and their families. This includes assessments, counseling, education, and case management to support the physical, emotional, and social needs of elderly participants. **What You Will Do:** + Provide individual and family counseling, support, and education on aging, dementia, grief, and more. + Conduct assessments and collaborate with the interdisciplinary team to create comprehensive care plans. + Offer resources and advocacy for participants and families, addressing mental health, finances, and legal needs. + Assist with participant intake, eligibility, and disenrollment processes. + Conduct home visits and coordinate care transitions between facilities and home care. + Promote collaboration within the team to ensure a seamless participant experience. **Minimal Qualifications:** + Master’s degree in Social Work and 2 years of experience, including work with elderly or frail populations. + State license in Social Work preferred. + Experience with Medicare, Medicaid, and other social service systems. + Valid driver's license and reliable transportation required. + Strong communication, organizational, and problem-solving skills. + Ability to work in a collaborative and fast-paced environment. **Position Highlights and Benefits:** + Meaningful work providing vital support to elderly participants and their families. + Join a team that values compassion, teamwork, and professional growth. + Opportunity for training and development in a supportive, innovative setting. + Comprehensive benefits package, including health, dental, and vision coverage. + Paid time off, retirement plan, and opportunities for career advancement. **About Us:** Trinity Health PACE is dedicated to providing comprehensive, coordinated care to elderly individuals in the community.
