**Overview**
M Health Fairview- The University of Minnesota Medical Center has a 0.6 FTE (3 days/ week; Wednesday through Friday) opening available for a MSW on our Cystic Fibrosis team. There is an every 6th weekend and 1 holiday per year component to the position working inpatient.
The role of the cystic fibrosis (CF) social worker involves supporting adults with cystic fibrosis, as well as their families. The social worker will provide essential emotional, social, and psychological support as patients navigate the challenges of living with a chronic illness. As a key member of the CF care team, the social worker will collaborate closely with providers, nurses, respiratory therapists, and other healthcare professionals in both outpatient clinic and inpatient hospital settings. The social worker will assist patients and families in coping with the impact of CF on their daily lives, providing support in areas such as grief and loss, adjusting to life with a chronic illness, managing depression and anxiety, and helping with employment-related challenges. The CF social worker will also play an active role in quality improvement initiatives, program development, and educational opportunities within the CF community.
**Responsibilities Job Description**
+ Provide psychosocial support to CF patients
+ Work as an integral part of a multidisciplinary team
+ Support patients and families in adjusting to life with a chronic illness, including addressing grief, loss of a “normal” life, and managing mental health concerns such as depression and anxiety
+ Assist patients with navigating employment, health insurance, and life transitions
+ Provide counseling, crisis intervention, and referral services to patients and families
+ Actively participate in quality improvement initiatives aimed at enhancing the care and well-being of CF patients.
+ Contribute to program planning and development, working to improve services and outcomes for the CF community
+ Participate in local and national educational opportunities related to CF care
+ Engage in national CF care initiatives and committees focused on the role of social work in the CF population
+ Lead a CF Support Group
+ Work as a mental health coordinator, providing brief interim counseling and referring patients to mental health supports that specailize in chronic illness
+ Follow a caseload of CF patients that are post lung transplant in the community
**Qualifications**
**Required:**
Master's degree in Social Work with medical experience
Current LGSW Minnesota Social Work Licensure
**Preferred:**
Current LICSW Minnesota Social Work Licensure
