Social Worker MSW - Behavioral Health

Under general supervision of the manager of care management, the Social Worker is to work with and support the providers through the provision of resources, advocacy and referrals that address patient's social determinants of health.  The Social Worker is also an internal resource for community referrals and linkages.  They will partner with the interdisciplinary team (Behavioral Health, PSAR, pharmacists, PCP's, RN Care Managers) to help patients access appropriate care.

Required Qualifications:

Master's Degree Social Work Upon hire: Washington Advanced Social Worker License Or Upon hire: Washington Advanced Social Worker Associate License Or Upon hire: Washington Clinical Independent Social Worker License Or Upon hire: Washington Clinical Independent Social Worker Associate License Experience in Case management. Social Worker must possess current knowledge of the bio psychosocial aspects of illness, disability and death and be able to apply this knowledge in a professional and understanding manner in direct social work services to patients and their families and in collaboration with all health care professional staff.  Social Workers must have the ability to communicate well and interact with multiple health care professionals, including medical and allied staff. Organizational, problem-solving skills, and communication skills, as well as the ability to work independently as a team member. Some positions may require specific experience depending upon area of hire.

Preferred Qualifications:

Experience working in an out-patient setting.
