#1140 Geneva, NY, USA Pay Range $34.74 - $38.39 Job Description Provides direct therapeutic casework, counseling, and advocacy services to dialysis patients over the age of 16. Involves family or other members of the patient's support system in the therapeutic and continuing care plan as needed and appropriate. Provides education to patients/families regarding treatment modalities and reviews options in collaboration with the physician and nursing staff. Assesses the psychosocial, emotional, rehabilitative and financial needs of patients and families and collaborates with the multidisciplinary team to develop a plan to address these needs. Coordinates or follows up on appropriate referrals to other agencies for assistance with financial or clinical needs. Maintains appropriate documentation per professional and regulatory requirements. na
