Investigates, assesses, and plans interventions to help patients cope with social, emotional, economic, and environmental problems.

Reports to:  Director - Case Management

Interviews and assesses patients and/or patient's family, caregivers, and/or legal representatives.Drag Edit DeleteDetermines, prioritizes, provides and/or arranges for needed internal and external services/interventions.
Drag Edit DeleteParticipates in case reviews to evaluate case management and progress. Consults with healthcare team members to promote, monitor, and evaluate compliance with patient's treatment plan.
Drag Edit DeleteAssists with discharge planning and processes. Identifies appropriate resources, including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the discharge plan.
Drag Edit DeleteSupervises and/or trains new staff, students, and interns.Drag Edit DeletePerforms Medicaid screenings as indicated for NHP.Drag Edit DeleteAssists patients with Safety Net applications when needed.
Minimum Education
Master's degree in Social Work - RequiredDrag Edit DeleteRequired Skills

Certifications:

Three years of clinical experience in as a Social Worker in acute care setting - Preferred
