Continually identifies and promotes interventions that will result in the optimal level of independent functioning for each participant. Responsibilities: EEDUCATION + Master of Social Work (MSW) required EXPERIENCE + One year social work experience required + Two years' social work experience in a healthcare or related setting preferred + PACE social work experience preferred + At least one year experience working with the frail elderly KNOWLEDGE, SKILL AND ABILITY + Effective communication skills including the ability to assist team members in understanding participant's psycho-social profiles + Ability to develop and maintain relationships and to work productively with all levels of personnel including physicians, families and care givers + Strong verbal and written communication skills + Customer focus + Accountable/Results oriented + Integrity and respect for others + Possess active listening skills, analytical abilities and teaching abilities + Demonstrated commitment to work within the team of health professionals, accepting and considering all points of view + Must have basic skills with Microsoft office products and other software as needed WORKING CONDITIONS + Normal heat, light and safe working environment: typical of most office jobs + Clinical setting + Travel between locations and participant's homes will be required REQNUMBER: 34960
