and are committed to providing quality healthcare to our communityJob SummaryThe Womenrsquos Health Regional Social Worker contributes to the overall success of Backus Hospital Windham Hospital and the Hartford HealthCare system Performs the social work functions of bio psycho social assessment short term crisis intervention supportive counseling and resource referral Provides intervention to assist patientsfamilies to improve social and economic difficulties that may interfere with health and wellness of the mother and child Facilitates planning across the continuum of care and collaborates with the inpatient social worker for continuity of careKey Areas of Responsibility Working in collaboration with the Womenrsquos Health Team and inpatient Social Worker to conduct assessment and re assessment of patientsrsquo bio psycho social needs considering their premorbid status current as well as availableresources and the patientsrsquo and familyrsquos wishes Documents actions Working in collaboration with the Womenrsquos Health Team formulates a documented plan to meet the patientsrsquo needs including and communicated to the interdisciplinary team the patient andor responsible party and third party payers Revises the plan based on changes in patientsrsquo condition Working in collaboration with the Womenrsquos Health Team facilitates the discharge plan including the patient families and treatment team making any needed referralsarrangements and documenting actions Working in collaboration with the Womenrsquos Health Team facilitatesparticipates in patientfamily conferences as needed Provides crisis intervention and brief supportive counseling Assesses patients for Advance Directive status and follows up according to hospital policy Assess patients for possible abuseneglect and complies with mandated reporting guidelines in accordance with statutory guidelines and hospital policy Facilitates completion of state mandated screening requirements for patients being discharged to skilled nursing facilities Administers follow up copy of Important Message from Medicare in accordance with hospital policy Facilitates resolution of legal issues as indicated and appropriate including but not limited to conservatorship Facilitates completion of any applications for entitlements andor available resources Work where every moment field requiredExperienceOne year of social work or human services experience required Two or more years of health care experience preferredLicensure Certification Registration State of CT LCSW
