About the Unit

Corewell Health Helen DeVos Children’s Hospital (HDVCH) is developing a new 12-bed pediatric medical-psychiatric unit. This unit will accommodate pediatric patients who have co-occurring behavioral health and medical needs. Through the coordination of medical and mental health services in this dedicated inpatient space, HDVCH will be able to meet the needs of our community’s most vulnerable children. In addition to having the new unit, the behavioral health team will work with external community partners such as Pine Rest and Forest View to find placements for patients who come to our Emergency Department with behavioral health needs, but do not need medical care. With true collaboration, external partners will be able to transfer patients to HDVCH if they require care for both behavioral health and medical needs.

Scope of work

The Social Worker on this unit will utilize clinical judgement and expertise to provide assessment and ongoing care management/behavioral health interventions. The work will include ongoing contribution to the behavioral health plan, communication with the patient and family regarding progress, and management of unit-based group therapy up to 2 times per day. This role will work to develop strategies to ensure cost effective, quality outcomes to patients with complex care needs, including assessment and management of medical discharge needs. The Social Worker partners with the psychiatric and medical providers to continually assess, monitor and provide intervention related to the behavioral treatment plan. Discharge planning will include appropriate coordination with community agencies for follow up care. The Social Worker will participate effectively in both provider rounds and care conferences/family meetings as appropriate and ensure that all daily Care Management related written documentation is completed.

Qualifications

2 years of relevant experience related field RequiredLMSW- Master Social Worker- State of Michigan License Upon Hire requiredBasic Life Support (BLS) - AHA American Heart Association Only 60 Days required OrBasic Life Support (BLS) - ARC American Red Cross Only 90 Days required
