**Building Location:** Building A - Duluth Clinic - 1st Street **Department:** 46090 Population Care Management **Job Description:** This position will be responsible for performing complex psychological/social assessments, which include barriers to coping, mental health history, cognitive development, adverse developmental events, resistance to treatment, mental health symptoms and learning and /or mental health threat, residential stability, health system impediments, social support and vulnerability. Collaborate with the Healthcare team to address Social Determinants of Health. **Education Qualifications:** A bachelor degree in social work from an accredited social work program. **Licensure/Certification Qualifications:** Education Requirements + Bachelor’s degree in social work from an accredited social work program.
