We want your knowledge and expertise! **Job Summary and Qualifications** The Social Worker (BSW) is responsible for promoting patient-centered care by evaluating psychosocial needs, providing community service coordination for identified patient populations. As a member of a multidisciplinary health team, the BSW acts as a liaison between the patient and community resources/post-acute care providers. The BSW creates and facilitates timely implementation of discharge plans and arranges post-acute interventions identified in the plan of care. The BSW facilitates patient movement across the continuum of care and identifies, addresses, and tracks barriers to patient throughput. The scope of this role includes primarily Med/Surg units. **What you will do in this role:** + Works collaboratively with RN case managers, acute care nurses, physicians, insurance companies, and other staff members to proactively identify and address patient needs. Maintains effective communication between members of the care team. + Identifies and addresses barriers to discharge and recovery, including financial, emotional, psychosocial, medical, and spiritual. + Performs thorough assessments and develops appropriate discharge plans for high-risk patients with discharge planning or psychosocial needs. + Obtains input from the RN case manager to ensure the discharge plan is appropriate and aligns with the patient care plan. Reassesses and modifies patient discharge plan as needed, in conjunction with the clinical team. + Participates in the interdisciplinary care planning process and performs proactive discharge planning. + Provides education to patients and caregivers regarding community resources and post-acute level of care criteria, benefits, and coverage. + Coordinates and facilitates meetings between the care team and patients/families/caregivers. Assists patients and families in making difficult decisions while honoring their preferences and values. + Engages in community outreach and program development to support patients/families/caregivers. + Provides expertise in post-acute services and community resources. + Demonstrates knowledge of patient rights, advance directives, ethical issues, and confidentiality requirements. + Performs other duties as assigned. **What qualifications you will need:** + Bachelor’s degree in social work required + Licensed Clinical Social Worker (LCSW), or Licensed Baccalaureate Social Worker (LBSW), or Licensed Master Social Worker (LMSW) + ​1+ years' experience or BSW internship required + ​Currently licensed as a Registered Bachelor of Social Work in the state(s) of practice, according to law and regulation. + ​Certification in Social Work preferred Corpus Christi Medical Center (https://careers.hcahealthcare.com/pages/corpus-christi-medical-center) is a **630+ bed healthcare system of hospitals in Corpus Christi** and the surrounding Coastal Bend community. _Bay Area_ is our full-service acute care hospital and offers state-of-the-art cardiovascular services, bariatric, GYN and robotic surgery. _The Women’s Center at Bay Area_ , with its NICU Unit, is also the home of our graduate medical education program. _Doctors Regional_ is our acute care hospital, with a 24-hour emergency department, cardiac catheterization labs, orthopedic and surgical services, and inpatient rehabilitation. _The Heart Hospital_ offers complete diagnostics, treatment, surgery and rehabilitation for cardiac patients. _Northwest Regional/Northwest Behavior Health Center_ provides emergency medical services, laboratory, imaging services and outpatient behavioral health services. _Bayview Behavioral Hospital_ provides inpatient and outpatient short-term treatment for psychiatric patients ages 12 years and older, as well as substance abuse treatment. _Northshore Emergency Center_ is a full-service 24-7 emergency department in Portland that offers outpatient laboratory, imaging and occupational services. _Radiation Oncology_ offers advanced technology for treating cancer with the latest and most specific radiation therapies available.
