Social Worker (PRN) - Seacoast

Founded in 1906, McLeod Health is a locally owned and managed, not for profit organization supported by the strength of more than 900 members on its medical staff and more than 2,900 licensed nurses. McLeod Health is also composed of approximately 15,000 team members and more than 90 physician practices throughout its 18-county service area. With seven hospitals, McLeod Health operates three Health and Fitness Centers, a Sports Medicine and Outpatient Rehabilitation Center, Hospice and Home Health Services. The system currently has 988 licensed beds, including Hospice and Behavioral Health. The hospitals within McLeod Health include: McLeod Regional Medical Center, McLeod Health Dillon, McLeod Health Loris, McLeod Health Seacoast, McLeod Health Cheraw, McLeod Health Clarendon and McLeod Behavioral Health.  

Maintains a professional image and exhibits excellent customer relations to patients, visitors, physicians, and co-workers in accordance with our Service Excellence Standards and Core Values.  

Collaboratively works with interdisciplinary team to ensure completion patient specific care plan and post-acute needs.  

Effectively collaborates with community resources to align patient with available resources. 

Assists with referrals for community based resources, as needed. 

Provides case management as needed for interdisciplinary/specialty programs. 

Assists with various populations across the organization to ensure proper resources and support.  

Provides an environment that demonstrates caring, compassion and respect for all patients, families, and visitors. 

Consistently handles customer complaints promptly and effectively when problems encountered. 

Utilizes proper escalation procedures when appropriate.  

Attends departmental in services and staff meetings. 

Performs all other duties as assigned.  

 

Qualifications/Training 

1- 3 years related experience, computer literacy, excellent communication and writing skills preferred  

1-3 years experience in a social work agency or hospital setting preferred 

Prior experience in a healthcare field or social service agency preferred 

1-3 years of related work experience preferred 

 

Licenses/Certifications/Registrations/Education 

Case management certification preferred  

Licensed bachelors or masters prepared social worker within the associated region of responsibility required 

Bachelor’s or master's degree required
