Solid Organ Transplant Licensed Alcohol Drug Counselor

This supervisor position is responsible for assisting the Abdominal Operations Manager with department operations, staff supervision and clinical support as needed for the pre-kidney, pre-liver, and living donor teams. **Responsibilities Job Description** As a member of the Solid Organ Transplant team, the Licensed Alcohol Drug Counselor will provide a wide range of services and interventions to assist individuals, families, groups or communities to restore, enhance, or protect their capacity for productive social functioning and coping with the consequences of their illness and health care status throughout the continuum of transplant care. As a member of the transplant interdisciplinary team, they complete a chemical dependency assessment at time of transplant candidacy evaluation to determine candidacy and risk of recidivism. LADC will determine clinical interventions, and initiate crisis evaluation of patients as well as assisting patients with short-term counseling, facilitating therapy. The individual is able to assume independent practice and takes a leadership role in patient care, program, and professional issues that they confront in their daily practice with minimal supervisory guidance. + Demonstrates a range of licensed alcohol and drug counselor interventions that include patient assessment in the transplant candidacy and treatment planning process, case management, counseling, crisis intervention, and advocacy in assisting patients and families reach their goals as it aligns with transplantation. + Completes comprehensive assessments as required. + Reviews results of all assessment components and develop a summary of needs. + Develop treatment plan, which addresses problems identified through assessment. + Makes referrals for additional services as needed and follow up as needed. + Facilitate outpatient substance use treatment group and/or psychoeducation group. + Takes an active role in facilitation of interdisciplinary communication as it relates to patients and families experiencing complex psychosocial issues. + Attends the weekly multidisciplinary Transplant Listing Committee meetings where s/he shares assessment of patients, including treatment recommendations and interventions. + Demonstrates knowledge and compliance with MHealth Fairview and, ethics, and standards. Maintains knowledge of, and complies with, all relevant laws, regulations, policies, procedures, and standards, including mandated reporting requirements. + Documents chemical dependency evaluation, progress notes, and care plans in the electronic medical record. + Formally and informally educates patient/family and health care team members regarding the LADC, pertinent patient care issues, and population specific information. + Demonstrates clinical competence in conducting substance use assessments, substance use treatment, and psycho-education groups. + Demonstrates the ability to advance evidence-based practice by utilizing standards, guidelines and pathways for care delivery.
