Job Summary
Provides consultative, facilitative, technical and education support to patients, families, physicians, care managers, finance departments and health care facilities with regards to post-acute insurance benefits and coverage and the types, appropriateness, levels-of-care and availability of local post-acute services in the service area. Provides formal and informal education, formal and informal consultative support, facilitates benefit verification and, assists customer groups in the placement of patients in the appropriate post acute setting. Serves as a resource to the Spectrum Health Hospital Group, Spectrum Health Medical Group, Priority Health, Medical Staff, post-acute providers, vendors and the larger community. The consultant may provide coverage for, and be deployed in support of, Post Acute Community Liaisons.

Essential Functions
Serves as a post-acute services resource to the Spectrum Health Hospital Group, Spectrum Health Medical Group, Priority Health, vendors, post-acute providers, hospitals, Medical Staff and the larger community. Provides formal and informal education (CME, ANA or equivalent certified when relevant) regarding: nature, type and availability of post-acute services; best-practice clinical models; post-acute services role in improving patient outcomes and; role and value of post-acute providers in improving hospital, physician and community patient care metrics. Works closely with customer groups (hands-on when necessary) to help them understand, verify, qualify and quantify post-acute insurance and benefits coverage. Facilitates patient discharges to post-acute providers. Works closely with targeted Health Care Providers to help them understand which post-acute services will be of most benefit to their patients.Works with the manager to formulate and continually update provider lists.May provide coverage for, and be deployed in support of, Post Acute Community Liaisons.Is an integral part of the Marketing/Communications team and participates in such strategies that impact communication with the physician community.
QualificationsRequired Associate's Degree related fieldPreferred Bachelor's Degree related field2 years of relevant experience with persons requiring long-term acute care, sub-acute care, long-term care, neuro and rehabilitative services Required Relevant experience working with insurance companies and government payers Required Infusion therapy, home care and/or hospice experience PreferredLIC-Registered Nurse (RN) - STATE_MI State of Michigan required Or LIC-Physical Therapist - STATE_MI State of Michigan required Or LIC-Physical Therapist Assistant - STATE_MI State of Michigan required Or CRT-Occupational Therapist, Registered (OTR) - NB-COT National Board for Certification in Occupational Therapy required Or CRT-Occupational Therapy Assistant, Certified (COTA) - NB-COT National Board for Certification in Occupational Therapy required Or CRT-Speech Language Pathologist - ASHA American Speech-Language-Hearing Association required Or LIC-Bachelor Social Worker (BSW) - STATE_MI State of Michigan required Or LIC-Master Social Worker (MSW-Master) - STATE_MI State of Michigan required Or LIC-License Practical Nursing (LPN) - STATE_MI State of Michigan required LIC-Bachelor Social Worker Limited - STATE_MI #N/A SHCC required
