Responsibilities

Position Summary:

The Specialty Program Coordinator is responsible for the development coordination and maintenance of the care continuum for specialty patients. The Specialty Program Coordinator provides leadership for all aspects of the continuum of care beginning before surgery through 90 days post operatively. This includes: pre-admission testing peri-operative services inpatient unit and all other applicable entities within the applicable service line related to evidence-based practice. Assists in the planning development and implementation of program goals and operations. Coordinates and oversees the ongoing disease-specific certification process to continually improve care for the specialty patient population. Works collaboratively with physicians physician office staff care management home care programs and rehabilitation facility staff to promote a successful program and patient experience for specialty patients. Collaborates with multidisciplinary groups at the hospital to develop and provide patient education and coordinate care on an ongoing basis. Assists the patient and family in the smooth navigation of their needs throughout the care journey. Oversees the quality metrics for the program/s and works collaboratively with physician offices pre-operative testing and admission peri-operative and inpatient care areas to develop pathways/plans to address issues. Knowledgeable on specialty certification requirements and responsible for initial joint certification and ongoing re-certification.


Qualifications

Minimum Requirements:

RN license in CABachelor’s DegreeONC -- within 1 year of hireThree (3) years Medical/Surgical or Peri- Operative experience. Two (2) years nursing management/leadership/education. Experience in physician relations and patient experience/satisfaction.
